• Primarily adult/geriatrics patients, family practice candidates have opportunity to see pediatrics
• Scheduled is 40 hour work week - very flexible. Providers can start later or earlier or customize the work week to fill out the schedule.
• Days are 8 clinical hours and 1 hour for lunch.
• Rotated Saturdays - when working Saturday get a day off during the week. Sundays optional for additional compensation
• Hours available are 18-20, 30 or 40 hours per week (benefits start at 30 hours)
• Collaborative, team approach model, patient medical home model

Job Requirements:

• MD/DO degree from an accredited medical school and completion of accredited residency program

• Board Certified

• Valid NY State license, CDS, DEA, and BLS

• Understanding of and passion for Community Healthcare is preferred

• Bilingual language skills in Spanish preferred (not required)
• NYS, DEA, CSR preferred (active)
• New graduates completing training in the next 12 months encouraged to apply
