With locations in the Antelope Valley l offering integrated programs including primary medical care, behavior healthcare, outpatient substance use disorder including medication assisted treatment, and HIV/AIDS and Hep C services.
As a member of the Primary Care team you will see patients of all ages in a family care setting. Focusing on preventative healthcare and monitor HEDIS measures closely and offer on-going care and treatment for chronic disease management. In addition, You will treat mild to moderate mental health issues in the primary care setting.
