This position is located at the Ida G Israel Community Health Center at 2925 West 19th Street, Brooklyn, NY 11224
Responsibilities:
PAGNY Seeks a Primary Care Physician for full time employment at our Ida G Israel Community Health Center located at 2925 West 19th Street, Brooklyn, NY 11224.
- Provide comprehensive primary care to a panel of patients
- Supervise Internal Medicine residents in ambulatory care
- Collaborate with RN's and other team members on chronic illness management pathways
- Assist with Performance Improvement work to provide comprehensive care that reduces unnecessary hospitalization
Qualifications:
- Board Certified or Eligible in Family Medicine
- Experience with treatment of both adult and pediatric patients preferred
- Competence in clinical care and team leadership within the scope of Primary Care
- Excellent psychosocial skills
- Experience with an electronic medical record
