We are currently seeking a full-time Primary Care Physician to join our team at Polk Center Hospital located in Polk, Pennsylvania. This facility is located 1 hour north of New Castle, Pennsylvania.

Monday - Friday 8:30am to 4:30pm with 30 minute unpaid lunch (37.5 hours per week)

The Primary Care Physician provides medical assessments and evaluations, provides treatment for emergent, urgent, and routine medical issues, provides and supervises medical treatment for chronic medical conditions, and participates in organizational initiatives to help prevent diseases and injuries of patients in a correctional setting. The Primary Care Physician works under the direction of the Site Medical Director and/or the Statewide Medical Director, and collaborates with the Health Services Administrator to ensure that efficient and clinically appropriate medical services are provided.

• The contractor/physician will provide primary care physician services to 192 patients/individuals with intellectual disabilities.
• Act as a Primary Care Physician for individuals at the Center and provide physical examinations, record reviews, prescribe medications, write physician orders, etc. as medically necessary to meet individual needs and all state and federal guidelines.
• Actively participate as a member of the Interdisciplinary Team.
• This person will also provide on-call coverage at Polk Center. While on-call, the physician must be readily reachable via phone or beeper and be within 30 minutes of the Center.

• Minimum Qualifications:

  • The medical physician must have completed a residency program in Family Medicine or Internal Medicine, which meets the requirements of the American Board of Medicine or American Osteopathic Board of Medicine.
  • The physician must possess a current Pennsylvania medical license, DEA registration, and Basic Life Support Certificate.
