- Pertinent Facility Information: Family Practice
- Name Clear Instructions: No Name Clear.
- Coverage Dates (Long-term only): 01/2020 - Ongoing
- Shift Schedule: Mon - Wed 7:30a-8p and Th & Fr 7:30a – 5p; we are looking for FTE locum providers that can do a minimum of 3 weeks per month.
Call Schedule/Responsibilities: Typical Outpatient Family medicine practice
- Procedures Needed: Typical Outpatient Family medicine practice. Minor procedures (lump, bump, excision removal) not required.
- Type of Patients/Cases: Mix of all patient population with the majority being 18 and older. Patient payer mix is primarily Commercial payers, Medicare and about 10% Medicaid
- Patients per Day/Volume: 18-20 patient visits per day, entire clinic sees an average of 100 patients per day.
- Certifications/Qualifications required (to include specific board): BLS
- Board Eligible or Board Certified: ABFM or AOA BC MD/DO in Family Medicine OR Truly BE (less than 2 years out of training).
- EMR: Epic
- Support Staff: 2 other physicians and CMA,LPN,RN,APP.
