Continued growth at these Community Health Centers has created opportunities for 3 Family Practice Physicians to join our group. These facilities are the predominate primary care providers and currently provide approximately 80% of the primary care in the service area. As they continue to experience considerable growth, currently employing 45 Providers at 6 modern medical practice sites in Vermont, while providing approximately 140,000 visits to almost 40,000 patients annually.
Each site is a certified “Level lll Medical Home”, with a highly developed and supportive panel management and case management system in place. They have a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within these primary care sites.
They have a fully integrated electronic medical record and practice management system with robust interfaces with the local hospital. After hours call (triage only) is shared among sites. Call responsibility is minimal at approximately only 1 night per month. Adult inpatient care is provided by the hospital sponsored Hospitalist program, and pediatric inpatient care is shared on a rotating basis between the Pediatricians and Family Practice physicians.
