Job Description

The primary focus of the Family Medicine Physician is to provide care for all patients.

Details:

  • Permanent position
  • Predictable work schedule with no evenings or weekends required
  • Average of 15 patients per day
  • Outpatient caseload only
  • 100% Medicare Advantage patient base
  • Will work among other physicians, advanced practitioners, medical assistants, care coaches, and on-site EMR experts
  • Collaborate with colleagues on shared goals of improved patient health that will impact your individual patients as well as the broader population
  • Follow a value-based care model which includes treating current patient conditions to practicing preventative care to improve the future quality of life for our patients
