Description: Hiring a Family Medicine Physician for an innovative community-based health center that is affiliated with a top teaching hospital. This Federally Qualified Health Center provides care to under-served communities in New York City. Located in a beautiful technologically sophisticated facility, the center provides an enhanced medical home with comprehensive primary care in pediatrics, women�s health, adult medicine, and HIV/AIDS care to patients. Mental health and nutrition services are co-located and integrated into primary care. Health educators, case managers, data analysts and public health specialists are an integral part of the health care team.

Responsibilities:

  • Assess the physical and psychosocial health status of individuals and families through health and developmental history and physical examinations
  • Manage the care of patients in collaboration with other health professionals and members of the health care team
  • Record and maintain a system of patient�s progress notes
  • Prescribe medications, request laboratory testing and routine x-rays
  • Carry out and modify a pre-determined immunization
  • Determine the possible alternatives in health care settings for providing health care
  • Provide information to patients and families relating to patient health status
  • Make referrals to appropriate health care facilities.
  • Participate in the development and conduct of quality assurance
  • Act as team leader at mobile medical unit
  • Train and mentor medical residents

Education Requirements:

  • MD or DO
  • BC/BE in Family Medicine
  • NY State Medical License
  • DEA

Experience Requirements:

  • Open to new grads as well as experienced physicians

Desired Skills:

  • Bilingual (English/Spanish) is a plus
  • OB or Women�s Health Background is a plus
