Projected Opening Date for Centers is Fall of 2018.
Hours of Operation (please note if closed on holidays):

1. 8-8 M-F 8-5 Sat & Sun.
2. Closed Thanksgiving / Christmas

Anticipated work schedule/shifts per week: 3 to 4 per week (7 days per each 2 week interval). Average Weekends per equal 2 per month.

Type of patients/procedures presenting in clinic? Urgent Care – Acute Care only “no” chronic illness management.

EMR system currently used in clinic? Docutap

Do you have prescription services? Escribe

Current staff made up of how many? (Include physicians, lab tech, front/back, & etc.): dependent on volume. CA, 2 – 3 MA’s X-ray Tech & Doctor (midlevel’s and/or additional MA’s as needed based on volumes).

X-ray Read Procedure (e.g. all films sent for over-reads? Turn-around time on 2nd reads)? We have protocol for this. Doctor can send what he/she is not comfortable with. Turn-around few hours.

Production/Bonus Info: Yes Production and Customer Satisfaction bonus plans. If new location for the first year the Physician can enjoy a Revenue Sharing Bonus.

Buy-in or partnership? In the short term no but in the long term possibilities exist.

CME allowance: Paid for Up to Date

CME days: Paid for Up To Date

Vacation: 1 week each 6 months up to 5 years of service. 1.5 weeks each 6 months after 5 years of service.

Sick leave days: No

Sign-on bonus: No

Moving Expense Assistance: No but will entertain based on entire package.

Loan Education Assistance available? No

Health/dental/vision Insurance (paid for? doc only? Family): Yes $800 per month allowance for Physician for coverage in the BCBS network plus $2,000 a year in a TASC fund for family coverage and $1,000 per year for single coverage.
