Our client, who is transforming the lives of their patients with a proven health and behavior change care model, has an outpatient Family Medicine Physician opportunity in Rockford, Illinois. The model integrates behavior change coaching into primary care for improved outcomes. Providers work with a highly trained care team including health coaches and care guides.

- Manage only 15-18 patients per day
- Predictable workweek with limited call (phone only)

- $220k - $250 K base salary range plus outcomes-based bonus
