• Provider Requirements: Must see all ages, family practice setting, Suboxone or X Waiver is a plus but not required.

Facility Details: Dates/Schedule needed: M-F (Mondays 7-7, Tuesday-Friday 7am-4pm). Option to do a 12 Monday and make one of the other days a half day OR 7-4 Monday and alternating 10-7.

EMR: Greenway, but will transition to Epic

Patient Population: 17–25 patients per day, Patient population is lower income population. High number of substance abuse population. Acute Care. Advantage is building brand new facility in 2023.

Additional details:
Salary Range: 230k target.
Other benefits: Full benefits package including medical, 401k with 3% match, dental, vision and supp. 4 weeks of vacation, plus 5 days for CME. 3500 for license renewals and/or certifications.
