CHC, provider of health care services in Nassau County, is seeking a full-time Family Medicine Physician. 5 Health Centers in Nassau County providing primary and preventative care in underserved areas. The CHC is bringing a new kind of care to communities that need it most. The goal is to see that every Long Islander has a "medical home" — a place where people they know provide the kind of primary and preventative care that safeguards the health of the whole family. The Family Medicine Physician will be responsible for providing comprehensive medical care to patients by diagnosing, treating, and helping the prevention of illness, disease, and injuries.
