Medical Director Ocala, FL

We are looking for a Family Practice or Internal Medicine Physician to join our group, providing primary care services to patients in Assisted Living Communities and Limited Skilled Nursing Facilities full time around Ocala and the Citrus County areas.

We have an immediate opening for a Physician to join our team caring for high acuity elderly patients who have difficulty leaving the home to receive necessary Primary Care Services. The physician will be primarily responsible for providing in-person, on-site visits and televisits, to patients in need within our contracted areas. This is a Monday through Friday position with no primary call and no weekends.

Our dedicated team of experienced Physicians, Advanced Practice Registered Nurses, Physician Assistants, Podiatrists, and medical support staff care for patients across North Central Florida. Our care consists of a strong team approach, frequent visits and proactive preventative care, in order to reduce medical cost and patient hospital revisits. We have been proudly providing care since 2010 for elderly and disabled individuals in Central FL. We have proven that personal engagement, quality of care, and exceptional service results in high patient and staff satisfaction. Our model is built on weekly on-site visits by our practitioners, and strong communication with families, patients, and facilities to provide greater continuity of care.

Duties:

  • Patient rounding and management of patient care at Assisted living communities and limited skilled nursing facilities (18-20 pts/day)
  • In-person/on-site and televisits for primary care
  • Provide oversight and collaborative support to advanced practice providers (APRN/PA) providing primary care services to these same patients
  • Work closely with operations and support staff to ensure patient need are met in a professional and timely manner to reduce hospitalizations and improve quality of life for patients
  • Review Home Health and Hospice care plans and collaborate with other providers involved in patient care to ensure patient needs are met
  • Attend and participate in weekly web meetings for all staff
  • Meet company’s performance standards for Medicare’s Merit-based Incentive payment System (MIPS)
  • Complete documentation and bill progress notes for all patient encounters in EMR
  • Comply with home health regulation and guidelines as established by Medicare and company policy

Qualifications:

  • Internal Medicine, Family Practice, General Practice or Geriatric Medicine Physician
  • MD/DO with full unrestricted license to practice in FL
  • Active DEA license

Schedule:

  • Full time
  • Monday through Friday (18-20 pts/day)
  • No primary call, no weekends

Salary (range):

  • $180k- 200k per year (depending on experience and qualifications)
