Job Details:
- M-F 8-5
- Join 4 other physicians, 1 nurse practitioner, medical assistant, nurse care manager, clinical social worker, and office coordinator
- Part time or full time
- Must be ok with prescribing opioids
- Complex care, Incoming patients are invite-only
- Looking for someone who has experience treating patients with multiple chronic diseases and truly believes in the value of a team-oriented patient-centered medical home model
- Seeing patients 16 and up, 8-10 patients per day
- The phone call is shared 1 in 5
- EMR is E clinical works, Onsite infusion center
- BLS certification and a strong focus on customer service is required
- Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
