Job Details:

• Board Certified/Eligible in Family or Internal Medicine
• Part-time is 20-30 hours a week including some Saturdays
• 100% outpatient
• Full time may be available if preferred
• 8-12 patients per day
• Office offers primary care services, bariatric medicine, acute-sick care, etc.
• Collaborative team-based practice
• EMR is Eclinical works
• 1 physician and 3 mid-level providers
• Less than 10% medicare population
• Active IL License and DEA or ability to obtain
