Join an established Health Plan owned group practice of Home Health/ House Call NP and Physician providers in northern New Jersey serving geriatric patients in home, or assisted living facilities. Employed by a regional health plan with it's own in-house practice group. Carry a panel of patients and rotate through them during the course of the year as needed. Spending 30-45 minutes with each patient creates a strong and rewarding physician-patient bond. Visit the patients with Medical Assistant/ Phlebotomist. Fully supported labs, xrays, mental health, Podiatry, etc. No hospital work. Candidates with passion to help geriatric patients will be considered including NP, or physician with Family Medicine, Internal Medicine, or Geriatric training.
