SCHEDULE:
- Full-Time
- Options for 4-Day or 5-Day Schedule
- Monday to Friday, 8:00am to 5:00pm
- No Nights or Weekends
- Some On-Call (explained below)


COMPENSATION:
- $180,000 to $215,000 Base Salary, dependent upon experience

The practice provides care to patients in private residences, as well as facility settings such as ALFs and ILFs. They break down their territories into small geographic areas, so that providers spend minimal time traveling during the work day.

All physicians begin and end their days as the central office in Lyndhurst.


COMPANY PROFILE:
This home-based primary care practice has been seeing Medicare patients in the comfort of their own residences since opening in 2014. They currently serve 3 territories throughout the United States and are excited to welcome new team members to the New Jersey practice due to growth. The practice currently employs 8 primary care providers who are supported by dedicated medical assistants, office managers, social workers, wellness managers, and more.



POSITION DESCRIPTION:
Serve as a primary care provider for a dedicated base of Medicare patients.

Physicians carry panels of approximately 185 patients and see their patients regularly, spending 30-60 minutes with each patient at each visit.

This practice stresses that its focus is on providing high-quality care, with no pressure to the physicians of an RVU- or quantity-based evaluation system.

Physicians begin their days in the Lyndhurst, New Jersey, office, where they meet with their Medical Assistant and prepare for the field. The Medical Assistant does all driving and supports the physician during patient visits.

The new physician will see patients in the area of Lyndhurst, Newark, Jersey City, and Paterson. Trenton is included in the territory, but will be carefully scheduled, due to the distance. The overall territory is broken down into smaller zones for scheduling purposes so that, in a given day, the physician is travelling within a relatively small boundary.


In collaboration with a patient care team (PCT), physicians are responsible for:

- Providing person-centered care by eliciting patients’ values and preferences and structuring care plans to reflect these desires.
- Routinely determining patients' prognoses and structuring care plans, including advanced care planning, to reflect these prognoses.
- Routinely determining primary drivers of hospitalization risk and structuring care plans to minimize these risks.
- Achieving high performance on CMS and NCQA-determined Stars and HEDIS quality metrics.
- Providing care that is consistent with medical evidence, including identified clinical protocols.
- Identifying obstacles to care and medical decision-making and collaborating with the PCT to address these obstacles.
- Leading interdisciplinary team meetings.
- Ensuring execution of member care plans by the PCT through clinical personnel supervision and education and training.
- Determining the need for consultation from medical, surgical, rehabilitative, and/or psychiatric specialties.
- Determining the need for and facilitating care by an emergency department, hospital, skilled nursing facility, home health agency, hospice and/or community-based organization as appropriate and consistent with patients’ goals of care.
- Collaborating with the practice Medical Director and Practice Manager in efforts of continuous quality improvement.
- Collaborating with the practice Medical Director and Practice Manager in efforts to pilot new care delivery models.
- Supporting the organization's efforts to establish a positive image for home-based primary care in the medical community.

The practice utilizes the eClinicalWorks EHR system.


Currently, all physicians are on-call for their own patient bases. Calls are fielded by a triage line and only forwarded when necessary. The practice is currently reviewing the option to switch to a rotating on-call schedule, but the physicians have indicated a preference to manage their own patients, as the panels are small and on-call volume light.



REQUIREMENTS:
- New Jersey medical license
- DEA
- Board Eligible or Certified, Internal Medicine / Family Practice / Geriatric Medicine
- previous exposure to geriatric care, home health, or house calls
