This progressive, national organization is currently seeking an exceptional Physician in the Pueblo area. Be apart of an employed, multidisciplinary clinical team, consisting of Physicians, NPs, Behavioral Health, Dieticians, Pharmacists, Social workers, and Case Managers who focus on improving the overall health of complex patients. Reduce ED visits and hospital admissions for these fragile patients by managing their care in complete geriatric-focused setting, before they develop urgent issues.

In this role, providers see approximately 5-7 patients per day, M-F.
