Genuine Health Group is seeking a Family Medicine or Geriatric Medicine Physician to join our growing team. Genuine is a value based health care company located in the beautiful Coral Gables, Florida. Genuine Health assists physicians and health plans to move all of their Medicare patients successfully to value-based care, whether they are Medicare Advantage or traditional Medicare. As part of its Genuine Health at Home program under which nurse practitioners provide home-based care to chronically ill patients, Genuine operates a primary care clinic across from Coral Gables Hospital. The clinic is the former medical office of Alex Loynaz, MD, a highly respected primary care physician who practiced there for many years. Genuine recently acquired the practice after Dr. Loynaz passed away. The practice has a significant panel of Medicare Advantage members with both United (Preferred Care and Medica) and Simply Healthcare.

We are building the next model for managing care delivery that embodies traditional values, promises reliability, and embraces flexibility and technology. Through its clinical division, Genuine improves healthcare outcomes, achieves cost savings for the care of patients who receive traditional, fee-for-service benefits through Genuine Health ACO as well as patients in Medicare Advantage plans through Genuine Health Management, its management Service Organization (MSO).

We provide comprehensive, integrated primary care to more than 29,000 patients across Southeast Florida through a network of over 100 independent physician primary care practices. While these practices are generally caring for patients of all ages and in some cases across multiple specialties, the focus of Genuine is on the Medicare aged patients aligned to its ACO and MSO. Genuine assists these practitioners with care management, case management, and transitions of care including visiting patients at home by Registered Nurses and Nurse Practitioners.

The clinic is intended to serve as prototype of Genuine’s planned staff model primacry care centers throughout Florida. The physician will provide medical care to the Medicare Advantage members within the clinic. The physician assumes the responsibility of providing comprehensive general medical care, including consultative services, as deemed necessary for the patients of the clinic and patients assigned to Genuine’s MSO and/or ACO. The physician will also provide oversight of the Advanced Practical Registered Nurses (APRNs) who care for patients in their homes, through transitions of care, and in some cases in the clinic setting as appropriate. The physician will also provide oversight of the care management infrastructure which delivers at-home RN care and telephonic clinical care and non-clinical referral coordination. The quality of care will be in keeping with the standards of the appropriate national organization, the Medicare Advantage plans, and CMS with resulting cost savings through improved patient health, decreased inpatient admissions, and removal of duplication of services.

Responsibilities and Standards

• Elicits histories and conducts physicals.
• Provides proper medical care as allowed by medical training and licensure.
• Provides preventive medical care including health risk assessments and annual wellness visits.
• Participates in Transitions of Care and Avoidance of Admissions patient case conferences to provide direction and coordinate care with hospital medicine physicians, APRNs, RN Care Managers.
• Provides supervision, education and direct assistance to visiting at home APRNs, RN Care Managers, and CMA Care Coordinators for patients of the system.
• Maintains a complete longitudinal care records and oversees care management record.
• Consults with specialists when absolutely necessary overseeing the case.
