Who You'll Work With

Care Partners Medical Group is a partner with PACE (Program of All-inclusive Care for the Elderly) to redefine senior care. Each participant works with a team of medical experts to develop a care plan that is personalized and regularly updated to meet their evolving medical and social needs. Senior care is not ‘one size fits all,’ which is why each participant has a customized PACE experience to meet their individual needs.

Care Partners Medical Group Inland Empire PACE opened in 2014. The center features a winding hallway that includes historic California art. The center also has a clinical exercise program and a program for participants to learn more about the Internet and social media so they can stay digitally connected to friends and family. The outdoor patio is a participant favorite, hosting activities like morning Tai Chi, tea parties and afternoon gardening. Other popular events are karaoke and the annual three-day luau, which includes a Hawaiian barbeque, live Polynesian dancers, limbo, and more.

Position Summary

The Medical Director is responsible for leading, planning, directing, monitoring and improving the overall performance for assigned clinics to achieve excellence in the delivery of daily operations. The Medical Director engages staff and cultivates a positive clinics culture that prioritizes human kindness and patient-centric care. The Medical Director is accountable for operational, clinical, quality, and business development activities throughout the medical clinic.

  • Full time schedule: M-F 8am-5pm (on-call schedule on rotation)
  • Type of specialty: Geriatric (out-patient clinic)

Your Responsibilities

Managerial Duties (80%)

  • Oversees the development of assigned clinic and regularly manages improvement projects and is accountable for operational, financial, and business development activities.
  • Develops, analyzes, and implements processes and systems to enhance customer service across assigned clinic(s) and takes ownership of and drives needed improvement in all aspects of operations, including clinic productivity, patient satisfaction/experience, provider satisfaction/experience, community efforts and serves as the local representative for the area.
  • Plans, coordinates and effectively leads assigned clinic to meet patient care, administrative, operational, and support requirements
  • Utilizes administrative time to provide chart and utilization review on patient cases to ensure quality care is provided that is in compliance to protocol established by PACE
  • Participate within the interdisciplinary team in the formulation of Plans of Care for PACE participants
  • Presents input into, initiates and/or facilities new medical programs and processes in support of PACE’s strategic goals

Participant Care (20%)

  • Provide medical care to PACE participants, including the evaluation, diagnosis, and management of their medical conditions, including but not limited to taking of a medical history, performance of physical examination, ordering and interpretation of diagnostic tests, formulation of an assessment of condition and a treatment plan, writing prescriptions, ongoing follow up of case management
  • Document medical care in the participants’ medical record, in compliance with PACE documentation policies
  • Provide after-hours medical coverage as assigned by the Chief Medical Office to assure that a physician is available 24 hours a day, 365 days per year (Medical Director may be involved with establishing the schedule for said coverage

Your Qualifications

  • MD/DO degree
  • Postgraduate training in a primary care specialty, including but not limited to internal or Family Medicine and completion of post-residency training in Geriatric medicine preferred
  • Active CA medical license
  • Active DEA license, NPI number
  • Adequate malpractice coverage
  • Board Certification in Family Medicine, Internal Medicine, Geriatrics, and/or related
  • 5 years experience in related field and 3 years management experience preferred
