POSITION -Medical Director-Primary Care

seeking a Full time or Part-Time Medical Director to add to the team of a rapidly growing mobile medical practice. Physician in this capacity will be responsible to ensure excellent patient care, quality outcomes and a high level of patient satisfaction. 60% of Medical Director's time would be providing direct patient care and 40% would be allocated to administrative I management duties and supervision and training of our clinical team of providers.

THE ORGANIZATION

exclusive focus is providing in - home-based medical care throughout New Jersey to chronically ill high-risk patients and others with barriers or functional limitations to getting timely routine and proactive care. We have devised a unique approach to preventative healthcare based on thoroughly researched care plans surrounding several key disease areas which are adhered to via a comprehensive, multi-disciplinary and proactive care strategy. We have established a well-defined hierarchy of care in which nurse practitioners and medical assistants serve as the backbone and a team of physicians ably monitor their care plans and ensure protocol adherence. We have recruited over 8,500 patients registered in the 5-year period since inception and work with over 60 referral sources throughout the state including Skilled Nursing, Assisted Living Centers and Independent Living Communities.

ESSENTIAL RESPONSIBILITIES

  • Provide medical management leadership for our team of nurse practitioners and physician assistants
  • Oversee clinical operations and direction of quality program components and quality improvement
  • Ensure providers deliver excellent patient care with quality outcomes
  • Work with management and other Medical Directors to ensure effective use of company resources including appropriate provider utilization
  • Implement protocols and clinical guidelines to enhance Care Model
  • Manage clinical escalations and respond to patient questions and grievances when needed
  • Assist in recruitment of staff members and assessment of staffing needs
  • Assist in training and onboarding of clinical staff
  • Perform chart reviews and performance reviews of clinical staff
  • Foster a positive work environment and culture, set appropriate expectations and standards
