- Hours: Monday - Friday 8 AM - 5 PM

- Average Patients seen: Case Management for 40-45 patients
- Call Schedule: Rotate call once every 5 weeks
- Sign-On Bonus: $2500
- Compensation: Hourly rate $23-$24/ per hour
