IMPORTANT:

Provider will be required to privilege for multiple facilities and asked to be flexible with "which" facility they will be scheduled at.

**Temp privileges are offered, so credentialing can proceed quickly**

• IMMEDIATE Need is for 6 FTE. Minimum availability is 2 weeks per month preferably consecutive
• 3-4 months of coverage needed ASAP with option to extend
• Call Coverage (rotation to be decided pending facility worksite scheduled)
  • please input call availability & Call Rates within the Submission form
  • please add Holiday Rate within the Submission form and/or within the profile under bill rate
• Schedule M – F; 7am – 5pm - Site can do 5:8s, or any combination of 4:10s Monday-Friday, or 5:10s if desired.
• 40-hour guarantee
• The CRNAs won’t be on any call. They can pick up weekend AM shifts if they want (would need to be included in confirmation)
• CRNAs will be supervised
• General B&B cases. No hearts, No neuro, No pediatrics, No OB
• Travel and Lodging provided
