SETTING:

CTC Outpatient Clinic

RESPONSIBILITIES:
5 - 6 patients per day
Will provide in house med management of Methadone and Suboxone
Provider is primarily needed for intakes (0 - 6 per week) and will do physicals
You will have assistance from 2 LPN's
EMR is SMART

REQUIREMENTS:
Active Kentucky License
DEA
Must have Suboxone X waiver
Prefer local candidate
