• October 3rd, 6am – 12pm
• Possible future dates
• Average 5-6 patients per day
• Active KY License
• CTC outpatient clinic (driving distance to Louisville)
• Must have a Suboxone X waiver
• Primarily needed for intakes (0-6 per week) and will also do physicals
