Growing organization focused around providing integrated primary health care and behavioral healthcare looking to hire additional providers:

  • Provide primary medical care and comprehensive HIV/AIDS services
  • Evaluate and assess patients for: substance abuse and addiction, including opioid addiction, hypertension, diabetes, hepatitis, asthma, and HIV infection
  • Provide comprehensive HIV/AIDS primary medical treatment including, HIV testing and counseling, ongoing clinical HIV disease monitoring, HIV-specific therapies and prophylactic management
  • Provide wellness and nutrition education to all patients seeking treatment within the organization
  • Mental health services provided include medication management, detox, addiction/methadone treatment, individual and group counseling, dual-disorders counseling (substance abuse and co-occurring mental illness)
  • Organization also provide juvenile and adolescent services at various clinics and juvenile detention centers across NYC
  • Organization uses a PCMH, collaborative care model to treat the whole patient from a primary care, mental health and socioeconomic perspective

NY State license, DEA, CSR (active or applied preferred) but not required
Residency training in Internal Medicine or APRN preferably ANP

Must have previous experience with primary care, addiction services

Mental Health/Behavioral health assessment skills preferred

Bilingual language skills in Spanish preferred
New graduates completing training in the next 12 months encouraged to apply
