Seeking Pulmonary Critical Care Nurse Practitioner or Physician Assistant to join Pulmonary / Lung Cancer team in Cambridge, MA.

  • The two populations of focus are lung cancer and advanced lung disease.
  • For lung cancer, the provider will serve to coordinate communication and care across a multidisciplinary team including radiology, radiation oncology, pulmonology, oncology, and thoracic surgery, and pathology.
    • Coordinates multi-disciplinary team meetings and agenda with communication of recommendations to relevant members of the health care team.
    • Lung nodule management and patient follow-up and assistance through the continuum of care will be an integral part of the position and will include utilization of nodule management software.
    • The position may require assistance with the development, coordination, implementation and evaluation of community outreach events, including physician and patient communities.
    • Works independently to review and assess data related to the program, including process and outcome metrics; maintains these metrics for monthly Oncology Steering Committee meetings and agenda for the multi-disciplinary team meetings.
    • May participate in the research.
    • The provider will also provide clinical care in the areas of smoking cessation counseling, survivorship, shared decision making for lung cancer screening, urgent care, and inpatient visits where needed (post-operatively, admissions for complications of treatment, etc).
  • The second population of focus is advanced lung disease and will include triage of patients referred to the practice, transitions of care, co-management with a pulmonologist, inpatient care with check-lists and care coordination for wrap-around comprehensive care for typical chronic lung diseases such as emphysema, bronchiectasis, interstitial lung disease, as well as urgent care visits for patients of the practice.
    • Assistance with DME needs (oxygen, NIV, nebulizers) in coordination with the office nurse and DME specialist.
    • Preparing and maintaining education materials for patients and coordination of care with primary care, social work, case management, hospital care, quality, and specialty services will be essential functions.
    • Participation in the hospital based readmission committee and research efforts is also expected.
  • Provides clinical care with a focus on smoking cessation counseling and Shared Decision-Making for lung cancer screening, urgent care and inpatient visits with co-management of pulmonary patients.
  • Sees inpatients to provide care coordination and follow-up management, including transition of care follow-up visits and survivorship visits.
  • Provides education and information to patients and families, helping to make the care seamless, continuous and comprehensive. Topics include smoking cessation, lung nodules, lung cancer, survivorship, and typical chronic lung diseases such as COPD/Emphysema and Interstitial lung disease, bronchiectasis among others.
