Job Description

The primary focus of the Psychiatric Nurse Practitioner or Physician Assistant is to provide home-based medical care to chronically ill patients.

Details:

• Full-time, permanent
• Perform an initial comprehensive assessment on all newly enrolled members
• Develop a member management care plan upon enrollment
• Establish and maintain effective relationships with members and their legal representatives, staff, PCP’s, and other collaborating mental health providers
• Escalate and communicate any issues with severely mentally ill, complex mental health patients, and any active inpatient psych admission
• Understand and comply with long-term care policies and procedures
• Actively participate in weekly multi-disciplinary team meetings

Qualifications

• Current, unrestricted California Nurse Practitioner or Physician Assistant license
• Completion of accredited program in Psychiatric/Mental Health Nurse Practitioner (PMHNP) OR Psychiatric Physician Assistant Program
• Minimum two years clinical experience
• Knowledge of CPT, ICD-9, and HCPC codes
