The Behavioral Health Consultant supports the entire Interdisciplinary Team (IDT) inclusive of the longitudinal providers, nurse care managers and licensed clinical social workers through regular consultation or direct patient facing work through in-person, telephonic or virtual encounters. The Behavioral Health Consultant is the subject matter expert for behavioral health conditions and provides treatment recommendations to the care team in addition to educational support on diagnosing and managing behavioral health and substance use conditions.

Primary Responsibilities:

  • Provide weekly caseload consultation to licensed clinical social workers managing patients experiencing depression, anxiety, or substance use disorders
  • Recommend any behavioral health treatment adjustments through the case review process or in ad-hoc consultations, including medication recommendations and other interventions
  • Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the medical setting until patients can be engaged in specialized care as appropriate
  • Conduct direct clinical evaluation of patients with significant diagnostic or complex behavioral health conditions via in person, telephonic or virtual visits and implement care plans which may include prescribing and regularly event response to treatment through scheduled follow up appointments
  • Triage patient to the appropriate level care and collaborate with community prescribers to facilitate transitions to the appropriate level of care
  • Deliver behavioral health education to the Interdisciplinary team regarding treatment of behavioral health conditions and substance use disorders for patients with complex medical conditions

Required Qualification:

  • Licensed psychiatric nurse practitioner; completion of accredited program in Psychiatric/Mental Health Nurse Practitioner (PMHNP)
  • Demonstrated ability to collaborate effectively in a team setting
  • Excellent communication skills
  • Positive, flexible, and solution-focused attitude
  • Ability to quickly synthesize medical and mental health data and formulate effective and evidence-based clinical recommendations

  • 3+ years of popst PMHNP
  • Experience with consultation-liaison psychiatry
