Are you looking for a new and exciting position working in healthcare? A growing Patient Centered Medical Home, (PCMH) model clinic is seeking a highly motivated, compassionate, and dynamic clinical professional Full Time, Nurse Practitioner.
The Nurse Practitioner will focus on the management and improvement of health for patients with multiple chronic medical [Register to View] is responsible for a broad range of skills, clinical expertise and developing a plan of care for the member, in conjunction with the providers and the whole multi-disciplinary team. The Nurse Practitioner will be responsible for a panel of 200-250 chronic care ambulatory patients in the clinic. They will be responsible for working interdependently with other professionals to develop a strong care plan.
Education & Experiences:
Licensed Nurse Practitioner in the state.
Experience with patients who have multiple chronic medical conditions, i.e. CHF, COPD/Asthma, HTN, Diabetes, Chronic Renal Failure
ACLS certification
Knowledge of managed care and insurance processes is preferred.
Experience with the management of Medicare, Medicaid, and commercial populations.
Experience with using EMR and efficiency a must in completing EMR notes within 48 hours, PrimeSuite preferred
