Founded in 2014, this unique organization is a Medicare ACO that works with private practices to enhance management of their Medicare patients. The ACO believes that primary care should be preventative and proactive, and their providers tailor care plans to each individual - integrating preventative medicine, chronic disease management, behavioral health, medication management, and more. They provide seamless support to ensure this vulnerable patient base never falls through the cracks and that unnecessary hospitalizations are avoided. They offer in-home services, virtual appointments, clinic-and-office-based care, and urgent care. Their provider staff of physicians, nurse practitioners, physician assistants, RNs, LPNs, and MAs work in collaborative settings with full support and resources in every location.
POSITION DESCRIPTION:
The new Nurse Practitioner or Physician Assistant will be responsible for a small, dedicated patient panel of geriatric patients.
All patients are located in Suffolk County and the NP will see patients via a combination of telemedicine (working flexibly from home) and home visits (local house calls on a limited basis).
The Nurse Practitioner or Physician Assistant will:
- provide regular, proactive geriatric primary care
- respond to urgent patient needs in a timely fashion
- understand what drives patient disease processes
- work with a palliative care team when necessary
- actively work to reduce and prevent hospital admissions
- participate in in organizational meetings
- leverage all resources to keep patients happy, healthy, and at home
There will be limited on-call duties (phone triage only) that will be discussed and agreed upon collaboratively during the hiring process.
REQUIREMENTS:
- New York NP or PA license
- DEA
- 2+ years primary care experience
- 1+ year experience working with the geriatric population
- general understanding of ACO quality metrics
- must live in or near Suffolk County due to occasional travel requirements
