Job Summary
Functions independently in the outpatient clinic-based setting in collaboration with a supervising physician. Patients are referred at hospital discharge based on their readmission risk status. The clinic goal is to see all high risk readmission patients within 3-5 days of discharge over one month, before referring them back to their primary physician. Emphasis is on health promotion of a focused disease process of system.

Clinic housed on the campus of CHS Northeast in Concord, NC. Provider will partner with multi-disciplinary team including: Cardiac Advanced Practitioner, pharmacist, social worker, dietician, MOA, nurses and respiratory therapist.

Incumbent will work day shift treating recently discharged complex patients with multiple comorbidities including: COPD, pneumonia, diabetes, cirrhosis, and sepsis. Prior experience preferred.

Essential Functions

  • Performs initial and periodic medical history, physical examinations, and daily assessments.
  • Records findings of examinations, assessments and plan of care.
  • Performs minor office-based medical and/or surgical procedures.
  • Writes appropriate prescriptions and administers medications.
  • Orders and interprets daily laboratory and radiology examinations.
  • Determines treatment plan for patient.
  • Documents all patient encounters, records observations, interventions, and actions in a medically appropriate manner on the patient's medical record.
  • Acts as a patient advocate for the population served.
  • Attends, participates and contributes to group/practice meetings and development of policies and procedures when requested.
  • Acts as a self-starter in identifying new readmission risk criteria and actively participates in hospital committees as needed
  • Candidate must be comfortable working in a new clinic (opened January 2018) environment and leading the multidisciplinary team
  • Communicates discharge plan electronically to referred physician

Physical Requirements
Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English fluently and in understandable terms. Must possess intact sense of sight, hearing, smell, touch, and finger dexterity. Must have the ability to react and perform in stressful situations

Education, Experience and Certifications.

Physician Assistant
Graduate from an accredited Physician Assistant program required; Master’s Degree preferred. Certification from the National Commission on Certification of Physician Assistants preferred.
