TeamHealth has an excellent full-time opportunity for a palliative care nurse practitioner or physician assistant to join our post-acute care palliative services in desirable south Denver suburbs. Patient rounding will take place in skilled nursing and long-term care facilities. We ask that candidates have a minimum of 1 year of palliative care experience (post-acute care setting preferred). Strong communication and partnership skills are required. You should have previous experience providing palliative care education to both clinicians and patients/families.
