HealthCare Partners is looking for a Full-time Palliative Nurse Practitioner to join our team in the San Gabriel Valley. The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Position Details:

  • Nurse Practitioner to join our newly redesigned Palliative Care Program
  • Mon-Fri, 8a-5p, 40 hours a week
  • 50/50 working in outpatient setting and home visits
  • Home base office will Be in Arcadia, but covering greater SGV region
  • Average Patients Per Day: ~8-10 PPD
  • No weekends or nights required but must be open to being on call
  • Dedicated support staff


Requirements:

  • Master’s degree from a four-year college and/or a professional certification beyond a four-year college.
  • Certificate of completion from an accredited Nurse Practitioner Program
  • Unrestricted CA NP license
  • Current CA DEA certificate required prior to start date
  • At least 4-5 years of experience preferred
  • EMR Proficient
