JOB SUMMARY:

Provide palliative care for patients facing chronic, complex and/or life-threatening conditions and their families at various locations within the flow of patient care. Participates in comprehensive medical and psychosocial evaluation of patients and families, skilled and meticulous symptom management, coordination with discharge planning and continuity of care. Serves as clinical and educational role model and mentor. Provides care and support in collaboration with the primary care team. Ensure compliance with practice standards and codes, participates and organizes in care management conferences and family meetings, and coordinates with discharge planning.

Job Responsibilities:

* Conduct comprehensive, focused assessments of patient and families with palliative care needs and those with primary care needs in the nursing home setting, assisted living setting, and home setting.
* Ability to work between all aspects of the Palliative Care Team and the Nursing Home Care Team including Inpatient and Outpatient Palliative Care, Home Health, Hospice, Assisted Living Facilities and Skilled Nursing Facilities.
* Ability to adapt efficiently and effectively to the Palliative Care Department evolution and growth in order to meet the health care needs of the community and quality goals.
* Formulate, implement, monitor, evaluate and modify palliative care plans with input from palliative care team and other collaborating care providers.
* Work with patients and families to determine their understanding of their condition, proposed treatment plan and treatment options.
* Assist patient and families to develop self-care capabilities relative to identified problems.
* Assist in all facets of care coordination for palliative care referrals.
* Engage in quality improvement in patient care services and department functions, including evaluating the effectiveness of the health care delivery system in meeting the health care needs of palliative care patients and families throughout the health care continuum.
* Advocate for improvements in palliative care through participation in professional, private, and governmental groups.
* Provide clinical expertise and guidance for undergraduate and/or graduate nursing and/or PA students in the specialty area.
* Act as resource person for clinical information and education as requested by community agencies.
* Assess the environment for potential need for consultation.
* Plan clinical and educational interventions which are consistent with individual/organizational needs and goals, and evaluate change/outcomes.
* Implement patient and family education in collaboration with the interdisciplinary team.
* Define goals for professional growth, and participate actively in professional activities and organizations.
* Demonstrate accountability for evaluation of clinical practice, including adherence to organizational, state and national practice standards and codes.
* Engage in active and frequent self-care activities for personal and professional growth and longevity.

KNOWLEDGE/EXPERIENCE:

* Unrestricted license to practice in state and per Physician/APRN Standard Care Arrangement.
* Minimum of 5 years nursing OR PA experience preferred, including care of patients with complex, chronic and/or life-threatening conditions, and those at end of life; clinical nursing care in oncology, hospice, ICU preferred
* Hospice and Palliative Care Certification or eligibility desirable.
* Knowledge of medications and prescriptive authority per state law.
