About the Practice and their Mission

Astera Cancer Care seeks a Certified Nurse Practitioner or Physician Assistant to join our pre-existing Palliative Medicine and Supportive Care Program. The ideal candidate must be able to travel to multiple practice locations in Northern and Central New Jersey.

Astera Cancer Care is an independent and physician-owned multi-specialty community oncology practice serving 12,000 new patients annually in Northern and Central New Jersey. The group provides coordinated services across every stage of the continuum of care, beginning with the diagnosis of cancer to active treatment, cure, and survivorship. Our mission is to transform cancer care and the care and management of blood disorders with patient-focused, research-based treatment guided by compassion.

Astera Cancer Care is a proud partner of OneOncology. OneOncology is a national partnership of leading independent community oncology practices working together to improve the lives of everyone living with cancer with a physician-led, data-driven, technology-powered, and patient-centric model. Through OneOncology, partner practices have shared technology platforms that foster communication, data sharing, and clinical excellence across the network. OneOncology’s non-exclusive clinical trial site management subsidiary, OneR, delivers complex, multi-center clinical trials to affiliated practices.

About the Role

The Advanced Practice Provider (APP) will join our preexisting Palliative Medicine and Supportive Care programs as a specialist, assessing each patient's condition and developing a plan of care focused on improving the patient's experience living with cancer. The APP provides consultation and administration of specialty-level palliative care, pain management, symptom management, and supportive care to meet the needs of oncology patients at Astera Cancer Care. The APP serves as a professional and qualified provider with the responsibility to practice their profession commensurate with their licensure, training, and experience under the laws and regulations governing the practice of an APP in the state where services are performed and all guidelines of applicable professional and accreditation agencies. The APP is responsible for working collaboratively with patients, their families and other caregivers, the primary care physician, and other specialists, as appropriate, in an active practice to deliver episodic acute care and chronic medical management for patients with progressive illnesses.

Essential Job Functions:

  • Perform office-based and telemedicine palliative care visits utilizing informative, comprehensive assessment and evaluation for patients and their families.
  • Collaborate with the patient's medical oncologist, radiation oncologist, surgeon, advanced practice providers, and other health care team members to develop realistic care expectations based on goals of care.
  • Facilitate patient and family decision-making regarding treatment decisions and perform advance care planning visits.
  • Prescribe pharmacological and non-pharmacological interventions.
  • Monitor patient's response to treatment modalities and revise the plan as needed.
  • Assist in all facets of care coordination for palliative care referrals.
  • Communicate effectively with patients and families regarding the patient’s diagnosis, prognosis, and treatment plans.
  • Document and bill accurately for palliative care services provided.
  • Serve as a resource and educator to healthcare team members regarding palliative care, serious illness, and end-of-life care management.
  • Develop and maintain effective working relationships with providers, staff, leaders, and community organizations to promote a positive image, heighten awareness, and encourage collaboration.
  • Partners with the palliative care program director and practice leadership in the development and growth of the Palliative Care Program.
