Tower Health is seeking a Nurse Practitioner to support our team at Reading Hospitals Hematology/Oncology practice. The Nurse Practitioner will work closely with our group of Hematologists/Oncologists to manage patients in a team setting, as well as, function independently when needed.

JOB SUMMARY

The Advanced Practitioner functions as a primary health care provider with the responsibility of managing health related problems and conditions. Serves as a clinical leader and works closely with the physicians in managing outpatient care, ordering of medication, order necessary labs or tests, prescriptions and appropriate documentation of patient charts. This includes consultations for in person, office visits and phone consults with management or others as applicable. Works within specific department with physicians, surgeons and multidisciplinary healthcare professionals in coordinating and delivering patient care and facilitating communication between the patient and family members as applicable, various hospital departments, the Nursing Team and the Medical Staff on specific indicated issues. Functions as a clinical expert in the area; dependent on departmental assignment, may be required to have weekend coverage, inpatient consults, and rounds

  • (1) Ability to adapt job duties based on physician availability and office coverage; working closely with one physician in a team setting as well as covering other team members based on availability.
  • (2) Evaluation of patients in a pre-chemotherapy setting including dosing evaluation, assessment of performance status, and assessing and treating side effects; assisting in routine follow-up examinations for oncology and hematology patients.
  • (3) Assess response to therapy by evaluation of bloodwork, radiographic imaging, and clinical responses; assisting nursing in triage of phone consultations and evaluation of acute visits; performing emergency visits and assessing and treating acute infusion reactions to chemotherapy and non-chemotherapy drugs.
  • (4) Perform history and physical examinations and preparation of discharge summaries; working closely with the billing department and chart auditors on appropriate coding and insurance issues; perform hospital rounds with attending physicians to evaluate new consults and hospital follow-ups; performing bone marrow biopsies.
  • (5) Initiate and monitor Coumadin / anti-coagulation dosing; monitoring and adjusting hematologic and drug flowsheets; attend conferences and assist in the multi-disciplinary approach to patient care; refer patients appropriately to other specialties as well as work closely with cancer center support staff such as nurse navigation, social work, nutrition, and case management.
  • (6) Assist in the education of students and residents, and assist in training of new midlevel providers in the office.
  • (7) Proficiency and compliance in the electronic medical record; maintenance of appropriate licenses and training; subinvestigators for clinical trials; management of the physician and midlevel schedules; triage and collection of necessary information for new patient referrals; managing and ordering of blood products.

CLINICAL LEADERSHIP:

Performs clinical assessments and documentation on patients; rounds, assessment and documentation on in-hospital patients on departmental specific area when Inpatient. Serves a point person for staff, in the office to assist with clinical issues when physicians are not available.

  • (1) Guides staff in clinical decisions regarding patient care, assists in development & revision of protocols and other administrative tools used in patient care, communicates with other clinicians regarding the plan of care for patients.
  • (2) Performs initial and on-going assessments on patients; documents daily progress notes.
  • (3) Reviews telemetry, labs, x-rays and other diagnostic studies.
  • (4) Communicates and collaborates with surgeons and consulting specialists.
  • (5) Prescribes diagnostic and pharmacologic interventions and perform specific clinical procedures as detailed in approved Allied Health Hospital credentials.
  • (6) Diagnose and formulate therapeutic plans of care.

DISCHARGE PLANNING:

Collaborates with appropriate health care team members Discharge Planning for inpatients The Responsibility and Accountability 'meets standards' when: (1) Collaborates with Case Manager in establishing appropriate discharge plan for patients in a timely manner (2) Prescribes physical and or occupational therapy and collaborates with therapists on patient treatment (3) Communicates with family members (4) Completes appropriate discharge information prescriptions in electronic discharge management system (5) Dictates discharge summaries in a timely manner.


Candidates must be a graduates of an accredited Nurse Practitioner program with a current CRNP license in the state of Pennsylvania. Experience is preferred, but not required. The ideal candidate must have a strong desire to work in an Oncology environment.
