In this role, you will provide primary care to patients in long term care settings; coordinate with facility staff to deliver high quality care on site. You'll interact closely with the facility staff and providers throughout the community; educating, building relationships and showing leadership in how to best address the complex needs of the patient and family.

Primary Responsibilities:

  • Provide preventative and primary care for patients in assigned long term care facilities
  • Work with primary care physicians to provide the best care possible
  • Collaborate with the nursing staff and the patients' families
  • Work with the facility's utilization review process
  • Performs patient case management
  • Supports clinical services, conducts patient health assessments and obtains complete patient history, determines the diagnosis, and develops treatment plans including and prescribes appropriate medications if applicable

Requirements:

  • Solid knowledge of communicable and chronic diseases of public health significance, and normal/abnormal adult health and growth/development
  • Previous experience (minimum 1 year) with geriatric population in a clinical setting
  • NY License as an NP or PA required
  • Previous experience in I-SNP, HMO +
