Physician Assistant Long Island NY (Suffolk County)

Job Summary

Health currently has an opening for a full time, experienced Geriatric Physician Assistant with a minimum of three years clinical experience from a geriatric practice environment. This position will be located in Suffolk, Long Island NY. Position will entail working with complex sick Medicare patients via telemedicine and home visits. *APPLICANTS MUST RESIDE ON LONG-ISLAND* Home visits throughout Long Island will be done on a weekly-basis.

Responsibility:

The Physician Assistant position is responsible for providing medical services for the sickest patients of Health ACO within the scope of licensing, training and credentialing via a combination of telemedicine and in-home visits. The Physician Assistant role will require to have a deep understanding regarding what the root causes are for the patients’ illness. NP will have access to claims data from CMS, EMR data, and HIE data in order to really understand what is driving the patient’s disease process. NP in certain cases will need to be comfortable working with a palliative care team when necessary and leveraging all of the ACOs resources in order to keep the patient happy, healthy, and at home out of the hospital.

The Physician Assistant is responsible to the ACO Medical Director and Chief Medical Officer for all clinical practice activities and responsible to the Chief Operating Officer and Chief Executive Officer for all non-clinical activities. The position will implement the mission, philosophy and goals of health in all activities.

The Physician Assistant will participate in on-call duties and actively participate in Organization meetings and committees.

Requirements:

Qualified candidates must be Board Certified Physician Assistants with a valid NY Physician Assistant license in good standing with the state

Experience:

Demonstrated knowledge of ACO quality measures, ICD-10 and HCC coding for reimbursement of services. (Preferred) Geriatric Physician Assistant with a minimum of three years clinical experience from a geriatric practice environment.
