Details:

• Permanent, full-time position
• Provide independent care in nursing facilities, home visits, hospital visits, and acute care facilities
• Generate initial assessment and Care Plan
• Conduct Home Safety Assessment and Comprehensive Chronic Condition Assessment
• Travel throughout Bay Area required
• EMR: Epic

Qualifications

• Board Certified
• Valid California Nurse Practitioner or Physician Assistant license
• Minimum of one year of clinical experience in Geriatrics, Palliative Care, or Internal Medicine
• Home visit experience required
