Due to company growth and expansion, Essen Health Care has an immediate opening for Managed Long Term Care program at our Bronx location.

What you’ll be doing

Manage and provide education, chronic disease management and care to patients

Develop, monitor and evaluate care of our patients and collaborate with medical team.

Conduct assessment of patient and reassessments and will discuss findings and work closely with patient, family and physician, making referrals accordingly using community resources to ensure optimal member functioning and cost effective outcomes

Required Qualifications:
Associate’s degree or higher
Current, unrestricted RN license for the state of New York
2+ years of experience working as a RN case manager
Experience in long-term care, home health, hospice, public health or assisted living
Proficiency with MS Word, Excel and Outlook
Experience with electronic charting
Experience with arranging community resources
