COMPANY PROFILE:
Since opening in 2006, this geriatric primary care practice has served facilities (ALFs/ILFs/SNFs), as well as other group settings, in Englewood and the surrounding area. They employ a staff of 15 talented individuals, which includes 8 primary care providers (physicians, PAs, and NPs). This position is open due to growth.

POSITION DESCRIPTION:
Serve as a primary care provider for a dedicated base of geriatric patients.

See an average of 20 patients per day, typically visiting only 1 facility per day, where all patients are seen in a time-efficient manner. This number is adjusted when travel during the day is required to additional facilities.

A dedicated scheduler will coordinate each day's patient panel.

The clinical focus is on preventative medicine, as well as chronic disease management and general primary care. This is largely evaluation and management of a Medicare population.

The practice uses a mobile EHR.

While providers practice autonomously in the field, they are supported by a dedicated, highly-trained office team.

There is a limited, flexible on-call component. This is phone triage only and can be coordinated around the provider's personal calendar. Call volume is light.

REQUIREMENTS:
- New Jersey PA license
- DEA
- at least 2 years of advanced practice experience
