A Westchester County health services organization is currently seeking a licensed Family Nurse Practitioner (FNP) and/or Physician Assistant (PA) for a promising School-based position at one of their facilities. In this role, the School-based FNP / PA will be responsible for providing quality care to students.

Responsibilities:

The School-based FNP / PA will:

  • Provide high quality primary care to middle school age patients enrolled at the facility
  • Provide care at one of the facilities when the School-based facility are not open
  • Share in the responsibility of site administration with other members of the Healthcare team.
  • Provide comprehensive patient care and establish a family/school partnership to meet the needs of the patients on an ongoing basis
  • Identify, diagnose and treat actual or potential health problems through case finding, health teaching, and health counseling
  • Handle other duties, as needed

Qualifications:

  • 2+ years of relevant work experience
  • Bachelor's Degree in a related field
  • NYS licensed Family Nurse Practitioner (FNP) and/or Physician Assistant (PA)
  • Working knowledge of Electronic Medical Records (EMR) systems
  • Engaged in learning and continuous Quality Improvement
  • Passionate about providing high quality primary healthcare to low-income, under-served residents and those lacking access to healthcare
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Bilingual (English and Spanish)
  • Primary care experience caring for children in pre-kindergarten through grade 12
