We have immediate opportunity for new Physician Assistant to join our Post- Acute Care team based in Redlands Community.


Position Details:

  • Schedule: Monday – Friday, 8:00 – 5:00 PM
  • Call Schedule: 8:00 AM – 8:00 PM
  • Locations: Designated region including twelve contracted Skilled Nursing Facilities, assisted living care facilities and Optum clinics.
  • Admissions, assessments, round and manage assigned patients, procedures, follow up care, and discharge planning
  • Keep patient and patients’ families updated of medical care, patient education Documentation, order labs, referrals for Specialists
  • Average Case Load: 60 patients
  • Work closely with SNF Physician and SNFst team.
  • Act as Primary Care Float to provide cross coverage / patient care at Optum clinics or Urgent Care setting.
  • Comfortable treating patients of all ages, primarily adult and seniors
  • Work well independently, excellent verbal and written communication skills, time management skills

Requirements:

  • Master’s degree from a four-year college and/or a professional certification beyond a four-year college.
  • Certificate of completion from an accredited Physician Assistant program required.
  • Unrestricted CA Physician Assistant license
  • Current AANP Certification required
  • Current CA DEA certificate required prior to start date
  • Welcome Physician Assistant Grads – will train and mentor!
  • Access to transportation, a valid driver’s license, and car insurance
  • Travel to multiple SNF, LTAC and Optum clinics in designated region
  • Flexible, strong multi-prioritizing skills and time management skills
  • Computer / EMR proficient
