Geriatric Nurse Practitioner

The adult Geriatric Nurse Practitioner functions independently, but with Supervising Physician collaboration and back up. The NP/Physician Clinical team will visit a variety of skilled nursing facilities and provide medical management to skilled and custodial nursing patients. The NP will be responsible for periodic night call, as well as rotation on weekend call. Thorough understanding of managed care delivery systems and utilization management is essential.

Qualifications

• Master degree with emphasis in adult/geriatric program

• State certification as adult/geriatric nurse practitiner

• Strong background in geriatric and/or internal medicine

• Current CPR certification

• Furnishing number

• Demonstrated ability to work independently

• Managed care experience

• Experience in geriatrics and skilled nursing facilities preferred

Responsibilities

• Managing a complex caseload of patients. Performance standard is at least 350 required visits per month

• Rounding on skilled nursing patients as needed

• Monthly rounding on custodial patients

• Through and accurate medical documentation using SOAP format

• Review and order medications (including IVs), labs and other diagnostic testing

• Participate in night and weekend calls

• Participate in QI program and Peer Review meetings

• Participate in utilization management program

• Ongoing patient/family communication

• Proactive communication with case managers and PCPs to keep them apprised of patient care issues

• Appropriate CPT coding and daily submission of billing forms to office

• Timely completion of all medical records in accordance with facility and other applicable policies
