We are seeking an Advanced Care Practitioner to join our talented team!

Overview:

The ACP (ARNP/PA) acts under the direction and supervision of a center staff physician(s), and is administratively accountable to the Medical Director. The ACP provides direct patient care in accordance with DaVita Medical Group practice site policies, relevant Nurse Practice Acts, relevant state and federal regulations, and within the limits of their experience, training and privileges.

Essential Functions:

Recommends, orders, and evaluates diagnostic studies and therapeutic interventions. Conducts comprehensive or episodic health assessments of patients with acute or chronic health problems in order to establish medical diagnoses. Monitors medical, radiographic, laboratory and any other diagnostic parameters associated with patient care. Monitors therapeutic procedures, assesses and manages/modifies follow-up plans. Prescribes and regulates medications in accordance with Florida Statutes. Records and documents health appraisal data necessary to maintain and coordinate health care needs and maintain a medical record. Consults and refers to other health care providers as indicated by individual patient needs. Conducts patient assessments including triage; educates patients on preventive health care (to include family/next of kin) and acute care follow-up as needed. Diagnoses and treats emergency conditions in accordance with BCLS guidelines as indicated. Performs other duties as required

Education:

  • Graduate from accredited school of nursing and graduation from accredited program for ARNP/PA including preceptorship. Master’s degree preferred
  • Certification by an appropriate ARNP/PA specialty board
  • Possess a current and valid Florida state license
  • Florida State licensed Nurse Practitioner Certificate

Experience:

  • 1 – 2 years of experience in an adult/family practice preferred with significant adult/geriatric patient experience
  • Managed care experience a plus.

Knowledge, Skills, and Abilities:

  • Working knowledge of basic primary care principles, theory and practice and ability to stay current with recent developments.
  • Ability to perform approved clinical tasks including but not limited to: Evaluate and treat patient needs, emergency triage, order and administer prescribed medications, assist physician with examinations and treatments and work independently (within nurse practitioner scope of practice), prepare and apply dressings, instruct patient in health measures and self-care, prescribe medications within the scope of a nurse practitioner’s license.
  • Ability to document and report findings and observations.
  • See patients in a timely and efficient manner consistent with DaVita access standards (30 minutes for a new patient or complex problem, and 15 minutes for follow-up and routine care).
  • Ability to write prescriptions and make referrals for non-PCP care (specialty care, radiology etc) under the supervision of the center physician.
  • Physically capable of providing on-site emergency treatment in life threatening situations.
  • Must be able to travel between DaVita clinics and corporate (as necessary).
  • Strong patient assessment skills.
  • Ability to work independently but ask for physician assistance using good judgment.
  • Ability to read, write, speak and understand the English language fluently and document clearly in the medical record.
  • Ability to read, write, speak and understand the Spanish language is a plus.
  • Ability to interact and communicate professionally and effectively with patients, staff and vendors.
  • Knowledge of Allscripts a plus.
  • Ability to perform dermatologic minor office procedures preferred.
  • Ability to perform well-woman exams preferred.
  • Able to be eligible for DaVita provided medical malpractice insurance.
