Chestnut Hill Diabetes, Endocrine and Metabolic Associates, Tower Health Medicial Group, is currently seeking a full time BC Nurse Practitioner for their Endocrinology Department.

The practice includes a team of 2 physicians, Certified Nurse Practitioner and a Certified Diabetes Educator who work together to meet the needs of the Endocrine and Diabetic population.

Provide collaborative care and management of patients. Specifically, the Nurse practice will: perform thorough patient focused history and physical examination on new patients, including assessment, differential diagnosis and treatment plan; assign follow-up visits for established patients at appropriate intervals, gather data, make assessments and modify the treatment plan; perform daily hospital rounds, re-examine established patients, gather data, make assessments, modify the treatment plan, and titrate medications to achieve optimal patient outcomes; recognize complications as well as development of problems in patient status and take appropriate action to treat or refer; request assistance from collaborating physician promptly and appropriately when the clinical situation requires; refer to other team members and physician services promptly and appropriately when the clinical situation requires; communicate effectively both through written clinical notes in the chart and through verbal interchanges with other clinicians and nursing colleagues; respond competently, courteously and promptly to pages and request from nursing and interdisciplinary staff regarding changes in patient status; establish rapport and relate to patients and families in a competent, courteous, engaging and supportive way; participate and lead family meetings, as needed by Endocrinologist; facilitate discharge planning to ensure clear communication and continuity of care as the patient returns to the outpatient setting; attend to inpatient medical records in a timely fashion; provide clinical leadership for the development and implementation of new protocols and algorithms that may be needed periodically for state-of-the-art in patient care.
