Maxim Health seeks a physician assistant for a full time position in Vallejo, CA. The PA will possess expert, complex, clinical decision-making skills; triage and collaboration skill and will display a high-level of comfort with providing care in a non-traditional setting centered on the following key responsibilities:

• Facilitating the triaging of eligible, appropriate patients referred for a potential admission facilitated by Medically Home guidelines; obtaining additional diagnostic and specialty consultation when needed;
• Providing inpatient level acute care in the home setting in collaboration with the home hospital attending physician and RN Care Coordinator;
• Completing the patient referral and providing safe handoffs to the attending physician and RN Care Coordinator;
• Communicating with the patient and family concerning the Medically Home model, incorporating patient and family values and goals in the plan of care;
• Navigating patients and families across the continuum of the Medically Home Phases of Care.
• Navigating barriers for patients and those within the system to accomplish optimal patient experience and outcomes;
• Navigating patients and families across the continuum of the Medically Home Phases of Care.
• Promoting a collaborative relationship and effort between the attending physicians, RN Care Coordination team, and supplier providers; and
• Serving as a role model for professional nursing practice.
