The Practice

SSM Health St. Clare Hospital - Fenton, MO

  • Level I Time Critical Diagnosis STEMI Center

The Opportunity

  • Provides services to patients during transitions of care from the inpatient hospital setting to the patient's community setting (home and/or assisted living)
  • The Transitional Care Coordinator will evaluate patients before discharge, provide medical oversight, education on medication management, disease management, plan of care, and assist in care coordination activities
  • The PA/NP will work regularly with the physicians and nurses in our facility and will be part of the care team
  • Previous work experience in Family Practice, Inpatient, Ambulatory Care Center, Skilled Nursing Facility and/or Emergency Department preferred
  • Current state license a plus
