• Full-time (Weekday schedule offers work-life balance)
• 3+ years of CHF experience inpatient/outpatient must be comfortable independently managing assigned patients in a busy practice environment with minimal oversight from the supervising physician
• A mix of clinical and administrative responsibilities
• The NP or PA will play an important role in achieving program goals of reducing hospital readmissions, improve patient quality of life, support expansion of new treatment options for Cardio patients, improve access to care
• NC license in Hand
• CHFN Certification is a plus
